$10,000 life insurance benefit, 4% monthly benefit payout for long term care with a 25-month initial duration and an extension of an additional 50 months (75 months total LTC payout).

Employees working 21+ hours per week are eligible for this benefit. Applicant's residence, work location, and billing address must be within the United States. 

Eligible employees are automatically enrolled in this employer funded plan and have the opportunity to apply for additional coverage for yourself. Your spouse/domestic partner is also eligible to apply for their own policy.
